Clinical waste collection cost
Clinical waste contracts are priced on what you fill and how often it is collected, not on the size of the practice. Put in your own containers and round below for a yearly range, then compare it with the published tariffs and contract values further down. A guide, not a quote.
Per sack or container collected
£8 to £25
Typical range, excluding VAT, November 2026
Estimate your yearly cost
Bags and sharps bins are priced from the typical per-container range. Wheeled bins use a published council tariff, which gives one figure rather than a range.
Estimated yearly cost
£416 to £1,300
About £35 to £108 a month
13 collections a year, 4 bags or bins each, excluding VAT.

Private contractors set prices customer by customer, so a council's published commercial tariff is a useful outside benchmark. This one includes treatment. Note how much less hygiene waste costs to move than clinical waste in the same bin.

What NHS buyers pay for whole areas
Integrated care boards often buy clinical waste collection for every GP practice and pharmacy in their area. The award values show the scale of the market, not a rate for one practice. Smallest first, as published.
| Buyer | Contract | Published | Value |
|---|---|---|---|
| NHS Bedfordshire, Luton and Milton Keynes ICB | Clinical waste collection and disposal for GP and pharmacy sites, first four months (extendable to July 2027) | July 2025 | £113,956 |
| NHS Norfolk and Waveney ICB | Clinical waste collection and disposal for 150 GP practices, branch surgeries and pharmacy sites, initial four-month term | March 2025 | £232,592 |
| North of England Commissioning Support | Clinical waste managing agent service, five years | October 2025 | £520,214 |
| NHS Humber and North Yorkshire ICB | Clinical waste collection and disposal for GP practices and community pharmacies, one year | March 2025 | £735,320 |
What moves the price
Two practices of the same size can pay very different amounts. These are the reasons.
- Number of containers
- Most contracts price each bag, sharps bin or wheeled bin collected. Fewer, fuller containers cost less than many half-full ones.
- The stream
- Yellow, purple, red and blue go to high-temperature incineration and cost more than orange, which can go to alternative treatment. Tiger-bag offensive waste costs far less again.
- Collection frequency
- A weekly round costs more over a year than an eight-weekly one. Match it to how fast you fill containers and how much secure storage you have.
- Location and access
- Practices on an existing round in towns cost less to serve than remote sites, and stairs or restricted parking can add a charge.
- Contract length
- Longer terms can bring the price down, but watch for automatic renewal and notice periods.
- Extra charges
- Container rental, a yearly duty of care or compliance fee, consignment note fees, failed visit charges and audit fees. Ask for each on its own line.

Questions about clinical waste costs
How much does clinical waste collection cost a month?
Published comparisons put a single treatment room, beauty or aesthetics clinic at about £15 to £60 a month, and a standard dental package at £15 to £45 a month, before VAT. Care homes and large surgeries with weekly wheeled bins pay more.
How much does it cost to dispose of one sharps bin?
Typically £8 to £25 per container collected, depending on size and lid colour, plus the bin itself, which one council supplies at £6.63 for a small box and £12.16 for a large one.
Why is clinical waste so expensive compared with general waste?
It has to be carried by a registered firm on a consignment note, stored and tracked separately, and treated at a permitted plant, often by high-temperature incineration. General trade waste goes to recycling, energy from waste or landfill at a far lower rate per tonne.
Is VAT charged on clinical waste collection?
Yes, waste collection is normally standard-rated at 20%. Most contractors quote excluding VAT, so check before you compare. Practices that cannot recover VAT should budget on the gross figure.
How can a practice cut its clinical waste bill?
Move non-infectious waste out of orange bags into tiger bags, keep paper towels and packaging in ordinary waste, close sharps bins only at the fill line, and set a collection round that matches how fast you fill containers.
Do councils collect commercial clinical waste?
Some do, for a charge. South Derbyshire District Council, for example, publishes a 2026/27 tariff of £18.01 a sack and £52.75 a week for a 240 litre bin. Many councils collect only household clinical waste, so most practices use a private licensed contractor.
